Safe Ride Home/Cab Voucher:

1. Go to the Nursing Resource Office

   Room 115North (down the hallway across from ED elevators)

   Phone 708-684-5730 (41-5730)

   Vouchers are available at the Front Desk when you enter and the office is open 24 hours. 

2. Call the number at the top of the voucher, they will arrange taxi, and taxi should be at the front of the hospital within minutes. 

3. There are 2 boxes on the voucher form a resident can check - one box for sleepy, one for sick.  The taxi driver keeps the yellow copy, and resident gives the white copy to Vera.  The driver should not ask for any money.

    

Unfortunately, it's a one way ticket, so getting back to Christ is your responsibility!
