TONS OF PAPERWORK FOR THE REAL WORLD

In Illinois you need to apply for your Illinois License at the same time as you apply for Step III, either via paper or electronic application. Throughout these directions, I refer you to a woman at Continental Testing Services.

 I would recommend only making one call to her either at the beginning or the end to confirm the fees.  You will need to send 3 checks with the application one to CTS (for me it was $90 to get them to process the application) one to FSMB ($780 for 2013 to take the actual test) and another one to FSMB ($65.00 in 2013  to get USMLE to send them old scores). You can use credit card online, as well.

Overall Time line:

  1. Get application and assemble all the extra crap you need to send to CTS see below. FYI – you can start this early and hold onto it for when you are ready to apply as it takes about a month to get all the stuff together

  2. Send in everything to CTS and wait 4-6 weeks to schedule the test

  3. Schedule for a date 2 mos. Off (is there still an orange scheduling sheet?)

  4. Pass Step III, wait up to 6 weeks for results

  5. Send a processing fee to Springfield, wait 8 weeks. The cost of IL Physician License is $700.00 through 2018

  6. If you took Step III early you need to send Springfield another TN-MED form

  7. Get your IL Lic. and apply for your Controlled Substance License, $5.00 and wait 8 weeks

  8. Get your Con. Sub. Lic. Apply for your DEA number, wait 1 month

List of extra crap you will need to get to send to CTS:

  1. a passport photo for the Certification of Identity form

  2. photocopy marriage license if you took Step III under a different name

  3. the 2 notarized pages you need (EBAHR and form WH) (can wait on this one)

  4. a photocopy of your med school diploma

  5. Official undergrad transcript with school seal (request from college)

Fill in the 3 page Step III application

  1. Board code is 014; Licensing authority is Illinois

  2. Fee enclosed: Call and ask Continental Testing Services what the step III application fee is (they enclose a fee schedule but it changes and if you get it wrong they will make you re-apply)

3-5   Demographics

  1. Country code for US is 099

  2. Medical Education: write the name of your med school and country code is 099

  3. Most of us will check the 4th box and our program code is 33; fill in Christ’s information

  4. Specialty code for ER is 05

  5. USMLE ID number (Rose has this in your folder on your Step I/II scores)

  6. Rose also has the dates you passed the steps in your folder

  7. Address

  8. Check if you need special accommodations

  9. Check if you do not want your med school to know your Step III score

  10. Demographics

  11. The third page is your certification of identity

  1. USMLE number from your folder with Rose

  2. Licensing authority is Illinois

  3. Consult Rose for notary public at ACMC or use your banker (usually free notary service for accountholders)

-License Number:  This is the temporary license that ACMC gets for us when we start.  Rose has this number in your file.

        -okay honestly I couldn’t figure this out.  Not kidding I called the lady at Continental Testing Services said leave it all blank aside from the number of times I have taken this exam which should be filled in with 01.  I submitted this application with this part (a b and c) blank

TN-MED Form

1-4 Demographics

7-8.  Your temporary Illinois License number should be with Rose in your file and the issuance date will be on it

**Give this form to Rose and she will fill in the form and return it to you with a letter from Girzadas stating that ACMC has no seal.  Keep these two together

**FYI—This form is a pain in the ass.  If you find that things are moving slowly later on in the process it is probably because of this form.  You can take Step III at any time but you can only apply for a license if the form says 24 months completed.  The State of Illinois will not call you to let you know so if you send in the form before 24 months has been completed just be aware you will have to get another one to send to the State later  (need verification that this is current)

Request for Examination and Board Action Record – 2 page horizontal form

Section I:  demographics, use home address, leave FIN/NID/ECFMG number empty; Rose has your USMLE number in your folder

Section II:  Check Step I and II and the dates are in your folder that Rose has

Section IV part b:  Fill in the address of Continental Testing Services PO Box 100 LaGrange IL 60525 (you get 2 for the same price so if you want a report you can fill in your home address – I did this to see how long things were taking so I knew when I got mine at home so did CTS)

a.  3 page application to take Step III

b.  3 checks:  one for your test, one for them to get your USLME scores, made out to Federation of State Medical Boards; finally one for them to process your application (made out to Continental Testing services)

c.  Application Transmittal Form – this is a checklist

d.  4 page application for your Illinois license

e.  Form WH

f.  FCVS Physician Profile (this is the 2 page Request for Examination and Board Action Record) if you are using FCVS

g.  TN-MED form and letter from Harwood

h.  Copy of your med school diploma

FYI – You do not need AF-MED, ED-NON, 5thPath/SS, Cert of Licensure

After you have sent these into Continental Testing Services (I would do fed ex or reg mail so you can make sure it all gets there) in will take 4-6 weeks to process and they will send you a orange (need update)  testing sheet to schedule your Step III which will be 6-8 weeks from when you call.  Make sure you check on the scheduling window.  If you go outside of a 3 month window they will make you pay to re-register!!!

Getting your Illinois License

After you pass Step III the State of Illinois will send you a one sheet letter asking for more money to process something or other.  This fee has gone up from $300. to $700.00 until 2018.

Then is you don’t hear anything after 8 weeks has gone by call the state at 217-782-8556217-782-8556 and ask them if they are missing anything.  (FYI – you will be on hold far at least 1 hour, and don’t be sarcastic they WILL hang up on you.)  For me it was the TN-MED form which said 16 months rather than 24 months.  If your TN-MED form was filled out earlier get Rose to fill out another one for you and MAKE A COPY and send it to the state at Illinois Department of Professional Regulation 320 West Washington St Springfield, IL 62786.

Getting your Illinois Controlled Substance License

You can’t get this until you have your Illinois license.  Go to www.idfpr.com and under agency links click Professional Regulation then go to Applicant Services then select Physician, Licensed from the drop down menu and select written application and at the bottom of the page is Physician - Controlled Substance

a.  Check First Time Applicant Box

b.  Check Physician Box

c.  Make sure to use the same Home address you used for your IL license and use ACMC for your business location

d.  Phone numbers make sure to give Rose’s 708-684-5375708-684-5375 so they can go through her if they have any questions

e.  Fill in your new IL license # and circle ALL of the drug schedules **VERY IMPORTANT***

f.  Fill out sections IV. V and VI

Getting your DEA number

After you have your IL controlled substance license (which will be sent to the hospital) you can apply for your DEA number online

-        -http://www.deadiversion.usdoj.gov  and click on applications and online forms and click on new registration applications then on DEA 224.

-        you will need both your license  and your controlled sub #s and a credit card for a$731.00 through 2014!!!! Fee.

-        If it takes more than 2 weeks it is likely because Springfield has to send Chicago your proof of controlled sub license. Call  800-478-7630800-478-7630 where a very nice lady named Sandra works and will give you the fax number of the machine next to her desk (312-353-1371312-353-1371) and let you fax her your controlled sub license and she will give you your DEA number over the phone!!!
